Rooted in the power of connection, our IOP blends expert-led group sessions with skill-building and real-life problem solving to help participants regain stability and move forward with confidence. It’s the perfect complement to ongoing individual therapy and psychiatric care—offering the structure, support, and momentum needed for lasting change.

Our Transitions Program is geared to older adults and those living both independently and at health care facilities. The Transitions Program is specifically designed to help older adults (typically 65 and older) in rural communities who are experiencing symptoms of anxiety, depression, and other mental health disorders.
The Transitions Program and our adult IOP program offers comprehensive psychiatric assessment, three hours of group therapy four times weekly, up to one hour of individual therapy, family therapy, and support services. This program is intended to last six-twelve weeks based on client progress and needs.
